Job summary

Savant Senior Living in Downey, CA is seeking a licensed Physical Therapist for part-time on-site duties in the West Los Angeles area. You will provide physical therapy services per physician plans of care and collaborate with the care team to support residents’ functional goals.

Responsibilities include evaluating mobility, designing and directing therapy, educating residents and families, and documenting progress.

Qualifications

• Requires a bachelor's or master's degree in physical therapy.
• Licensed to practice as a physical therapist in California.
• Minimum two years of experience; home health preferred.
• CPR certification current.

Responsibilities

• Provide physical therapy services per physician's plan of care.
• Assess and evaluate patient progress and adjust plans.
• Educate patients and caregivers on therapy and prosthetic use.
• Document progress and communicate with physicians.
