Job summary

Responsibilities

• Coordinate transfers and direct admissions, communication with providers.
• Collect intake data from ED, clinics and referring facilities.
• Coordinate MD-to-MD acceptance calls and document them.
• Collaborate on patient benefits/eligibility with placement teams.
• Manage ED referrals and relay information to ED.
• Triage and prioritize patient information and acuity.
• Work with admitting, EVS, case management to facilitate admissions.
• Model Family Centered Care and customer service.
• Participate in department process improvements.
• Performs miscellaneous duties as assigned.
