Job summary

SSM Health Transfer Center is seeking a skilled RN to coordinate patient placements across regions, ensuring timely admission and bed utilization. You will work with nursing staff, case management, and physicians to optimize patient flow and discharge planning.

Requirements include five years of acute care RN experience, BSN or higher, and Missouri or compact-state licensure. Night shift, 36 hours weekly, with sign-on bonus eligibility.

Qualifications

• Experience with bed management processes and patient placement workflows.
• Strong collaboration with nurses, case management and physicians.
• Ability to utilize hospital information systems for patient movement.

Responsibilities

• Collaborates with nursing staff and multidisciplinary team to place patients in appropriate level of care.
• Organizes placement processes to meet patient load and avoid bed delays.
• Utilizes software for documentation of patient movement and communication with case management and registration.
• Assesses documents and reports with clinical information for placement decisions.
• Alerts case management to bed availability concerns and changes in status.
• Participates in daily bed meetings and contributes to discharge planning as needed.
• Escalates capacity issues and coordinates with physicians and supervisors to prevent backups.
