Job summary

SSM Health is seeking a Registered Nurse to lead patient transfers from various facilities into our Transfer Center. The role emphasizes rapid response to inquiries, accurate documentation, and coordination with receiving hospitals to ensure safe, timely admissions.

The ideal candidate brings five years of RN experience in acute care, oversees transfer processes, and maintains strong inter-facility relationships. This is a full-time, on-site position with rotating shifts and weekend coverage.

Qualifications

• RN license in IL/MO/WI per location.
• Experience in acute care hospital settings preferred.
• Strong customer service and documentation skills required.

Responsibilities

• Facilitates, tracks and documents all hospital transfers in a patient-centered Transfer Center.
• Responds promptly to calls with professional, patient-focused communication.
• Collaborates with facilities based on services, geography, capacity and approvals.
• Follows EMTALA-related protocols and policies.
• Communicates patient condition and ETA to prepare receiving teams.
