Job summary

UCI Health, part of UC Irvine, seeks a Transfer Center RN Coordinator to optimize patient access across UCI Health locations. You will partner with the Capacity Command Center to coordinate timely transfers, bed placement, and clinical decisions, ensuring the right patient, bed, location, and time.

The role requires 5+ years ICU/ED nursing, California RN license, BSN, and strong computer/reporting skills, with Epic experience and case management credentials preferred.

Qualifications

• Strong knowledge of Joint Commission, CMS, CDPH, and EMTALA requirements.
• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties.
• Must possess complete understanding of acuity levels for specific patient population.
• Must demonstrate excellent customer service skills.
• Minimum of 5 years recent nursing experience in Adult ICU or Adult ED.
• Current California RN license and BSN.
• Advanced computer literacy and ability to run reports in MS Office and Epic.
• Ability to manage multiple projects and maintain a fast-paced workload.
• Ability to collaborate across the Health System.

Responsibilities

• Facilitate safe, timely, and appropriate patient access into across UCI Health locations.
• Coordinate clinical decision-making and patient placement with CCC Medical Director.
• Communicate effectively with referring and receiving teams; document actions in systems.
• Support continuity of care and capacity management across enterprise sites.
