Job summary

HCA Healthcare, Inc. in Plano, TX seeks an experienced Transfer Center RN to coordinate patient transfers across facilities.

This day shift role requires 9am–9pm hours with weekend availability and emphasizes bedside bedside preparedness, patient safety, and effective communication with referring facilities and hospital staff. You will manage emergent to elective transfers, ensure appropriate bed placement, and monitor throughput with adherence to EMTALA and quality standards.

Qualifications

• Graduate of an accredited nursing program required; Bachelor’s degree in Nursing preferred
• Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred)
• RN License in the states where the HCA facilities for that Division are located – multi-state license required if Transfer Center covers more than a single state (may be obtained upon hire)
• Knowledge of hospital and clinical operations and patient care practices
• Strong problem solving, analytical and critical thinking skills
• Ability to establish collaborative working relationships and communicate effectively

Responsibilities

• Performs transfer center functions with centralized patient placement in a timely, patient-centered manner
• Coordinates transfer of emergent, urgent and elective patients to final destination facilities via a single call
• Facilitates patient throughput with focus on quality and efficiency, with follow-up to all participants
• Promotes teamwork and effective communication with referring facilities, physicians, and staff
• Ensures timely bed placement and accurate patient information for initial plan of care
• Maintains accurate data in patient records and center workflow systems
• Navigates and documents in all technology platforms as needed
• Assesses patient acuity using clinical judgment to support safe transfers
• Triages and communicates clinical presentations clearly to accepting physicians and hospitals
• Adheres to EMTALA and other regulatory requirements and quality initiatives
• Participates in daily bed management meetings and coordinates resource issues
• Recommends process improvements to create efficient patient-centered transfers
