Job summary

Boston Medical Center Health System is seeking a Transfer Center RN to coordinate patient admissions and transfers across facilities. You will act as a liaison among patients, clinicians, and receiving centers, ensuring safe transitions and efficient operations.

The role requires strong clinical knowledge, organizational skills, and effective communication with diverse stakeholders; schedule includes nights and every 3rd weekend, with potential off-shift coverage.

Qualifications

• RN with ASN or diploma; BSN commitment within four years.
• Minimum of five years of clinical nursing experience.
• Massachusetts RN license required.

Responsibilities

• Coordinate patient admissions from external and internal sources.
• Manage transfer requests based on medical urgency and bed availability.
• Serve as primary contact for physicians, nurses, and facilities.
• Review medical records for accuracy and compliance.
• Facilitate bed assignments and status updates.
• Provide clinical assessment and triage for safety and care level.
• Collaborate with case management and social work on complex transfers.
• Maintain accurate admission/transfer records for reports.
• Participate in transfer center initiatives to improve patient flow.
• Provide coverage for Off-Shift Nurse Supervisor as needed.
