Responsibilities

• Coordinate patient transfers to ensure safe, timely placement.
• Perform triage to determine level of care and subspecialty needs for transfers.
• Act as liaison between referring and receiving facilities with clear, documented communication.
• Collaborate with interdisciplinary teams for patient-centered transfer decisions.
• Document all transfer activities in EMR and transfer-center software; prepare monthly reports.
• Participate in quality-improvement activities to optimize transfer processes.
