Cayuga Health's Transfer Center Associate per diem role supports seamless patient transfers, coordinating with RNs, physicians, and EMS to ensure timely, accurate transfers between Cayuga Medical Center in Ithaca and Montour Falls.

The position emphasizes clear communication, meticulous documentation, and collaboration with care teams to optimize patient transitions and care continuity.

Qualifications

• High School Diploma or equivalent required.
• BLS (CPR) certification required.
• Strong organizational and time management skills.
• Functional understanding of EMTALA and CMS guidelines.
• Excellent customer service and communication skills.
• Experience with electronic medical records and payer authorizations.

Responsabilités

• Builds trusting, professional relationships with referring parties to address transfer needs.
• Coordinates and facilitates hospital transfers in compliance with EMTALA and policies.
• Collaborates with physicians, nurses, Administrative Supervisors, and Patient Access teams to secure clinical information and initiate transfer registration.
• Ensures accurate documentation of transfer requests, demographics, clinical details, provider information, and payer authorizations in the EMR.
• Coordinates transportation logistics (ground/air) and informs stakeholders of updates or changes.
