Job summary

Sentara Health in Norfolk, VA is hiring a RN Transfer Center Bedflow Coordinator for the Fort Norfolk Transfer Center. This part-time (18 hours per week), day-shift role is based at the Fort Norfolk Transfer Center in Norfolk, VA.

The candidate will coordinate transfers, collaborate with a multidisciplinary team, and ensure timely patient placement while maintaining a high level of clinical care in a fast-paced environment.

Qualifications

• RN license required or eligible
• Experience in ICU/ED or transfer center
• Recent acute care experience
• Ability to multitask, type with reasonable speed and take accurate notes
• Clinical knowledge and critical thinking skills to meet standards of care

Responsibilities

• Coordinate transfers for patient flow across the system network
• Collaborate with multidisciplinary team to facilitate appropriate and timely patient placement and/or transfers
• Communicate capacity management plans to nursing units, leadership, and physicians
• Apply clinical knowledge and critical thinking to meet standards of care
• Work in a fast-paced environment with a positive attitude

Requirements

• Registered Nurse
• Experience working in ICU, ED or Transfer Center
• Recent Acute Care
• Must poses the following skills: Ability to multitask, type with reasonable speed and take accurate notes.
• Transfer Coordinators assesses, plans, develops, implements, and evaluates transfers for patient flow throughout the system network. Coordinates and collaborates with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfers. Coordinates and communicates capacity management plan to nursing units, leadership team and physicians. Possesses clinical knowledge and critical thinking skills to meet standards of care required by the patient population. Must have the ability to work in a fast-paced environment with a positive attitude towards all professional contacts.
