Qualifications

• Five years managing large projects and budgets.
• Five years experience in public relations.
• Proficiency with Microsoft Word, Access and Excel.
• Public speaking experience.
• Driver's license required; PA state upon hire.

Responsibilities

• Initiates individual and group tobacco counseling and provides follow-up care through the cessation/treatment process.
• Collaborates with physicians, office practices, and other medical professionals for patient referrals.
• Participates in community outreach initiatives related to tobacco cessation.
• Schedules and triages services for the tobacco treatment program.
• Documents all patient encounters; maintains treatment programs and follow-up.
• Provides pharmacology recommendations and coordinates results with primary care provider.
• Performs outreach in-network and outside provider practices to establish cessation partnerships.
• Performs follow-up contact for inpatient tobacco cessation consults.
