Job summary

Lehigh Valley Health Network in the Pocono area seeks a Tobacco Treatment Program Coordinator to lead individual and group cessation efforts within the outpatient setting. You will collaborate with physicians, practices and other health professionals to refer patients and coordinate services in the tobacco treatment program.

The role emphasizes patient assessment, documentation, data collection and outreach to build patient referrals and partnerships with in-network and external providers.

Qualifications

• High School Diploma/GED is required.
• 5 years managing large projects and budgets and 5 years experience public relations.
• Knowledge of Microsoft Word, Access, and Excel.
• Experience managing grants.
• Public speaking experience.
• DL - Driver's License_PA - State of Pennsylvania.
• Upon Hire and CTTS - Certified Tobacco Treatment Specialist within 1 year.

• Documents all patient encounters.
• Maintains individualized treatment programs and follow-up care.
• Performs follow-up contact (telephone or direct mail) for inpatient tobacco cessation consults.

• Provides pharmacology recommendations for patients and coordinates results with primary care provider.
• Carries out necessary tasks for triage and data collection.
• Ensures assessment and treatment are appropriate to the age, education, and culture of the patient and families.

Summary

Initiates individual and group tobacco counseling and provides follow-up care through the cessation/treatment process. Collaborates with physicians, office practices, and other medical professionals for patient referrals. Participates in community outreach initiatives related to tobacco cessation. Schedules and triages services for the tobacco treatment program.

Preferred Qualifications

• Completion of a tobacco treatment program through one or more recognized tobacco treatment centers that are ATTUD accredited.
• 1 year of experience with tobacco cessation program activities or related counseling (addiction).
• CTTS - Certified Tobacco Treatment Specialist - Granted by Recognized Issuing Authority within 60 Days

Additional Qualifications

• Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
