Job summary

Tri-County Mental Health, doing business as Beacon Mental Health, seeks a Care Coordination/Benefits Specialist to support TMS services and assist clients with MoHealthNet applications in Kansas City, MO. You will work with the clinical team to help clients access insurance and community resources.

The role requires knowledge of MoHealthNet, strong communication, and the ability to manage multiple tasks while maintaining client confidentiality.

Qualifications

• Education/experience combination of 4 years required.
• High School Diploma required
• Bachelor’s Degree preferred
• L1 Medication Assistant OR 1 year MA in ambulatory/outpatient setting
• MA certification must be obtained within six months of hire date
• Active driver’s license and minimum state-required auto-insurance
• CPR/First Aid certification

Responsibilities

• Contacts referred consumers to assist with MoHealthNet and benefits applications.
• Assists consumers with accessing community resources (housing, food, medical care, transportation).
• Tracks progress of benefits applications and provides outreach for MoHealthNet enrollment.
• Educates consumers about MoHealthNet benefits and processes.
• Coordinates services to secure other needed resources and follows up.
• Coordinates with Crisis Team for crisis interventions.
• Maintains communication with treatment teams and agency systems.
• Maintains progress notes in AVATAR within 72 hours of service.
• Promotes consumer wellness and data-driven care coordination.
• Attends weekly team meetings and completes assigned tasks.
