Job summary

Beacon Mental Health is seeking a Care Coordination Assistant to support the Healthcare Home team. You will help clients apply for MoHealthNet, schedule appointments, and connect them with essential resources and community services.

You will engage in outreach, track client data, and collaborate with multidisciplinary teams to ensure timely access to care. A driver’s license and CPR/First Aid are required.

Qualifications

• Experience in healthcare coordination or case management preferred.
• Knowledge of MoHealthNet and Medicaid guidelines helpful.
• Ability to research and connect clients with community resources.

Responsibilities

• Assist clients with MoHealthNet and benefits applications.
• Schedule appointments and coordinate services across teams.
• Provide resources and outreach to clients and communities.
• Track client data and generate status reports weekly.
• Maintain documentation in AVATAR and other systems.
