Work Schedule: 100% FTE, Day Shift role. M-F 8am - 5pm. You may be eligible for a $20,000 sign-on bonus. Work at the Headache Clinic in Rockford.

Qualifications:

• Graduate of an accredited Physical Therapy program or foreign equivalent (as determined by Illinois Department of Professional Regulation)
• Participation in ongoing educational programs and mandatory CEUs

Licenses & Certifications:

• Current Illinois license or in the process of acquiring licensure
• CPR certification
