Carrollton Springs, part of Lifepoint Health, seeks a Therapist for its Partial Hospitalization Program (PHP) in Texas. This full-time role delivers individual and group therapy, participates in discharge planning, and coordinates care with an interdisciplinary team.

Qualifications

• Master's degree in Social Work, Counseling, or equivalent is required.
• Current clinical or social work license as required by state regulations.
• CPR and De-escalation certification required or obtain within 30 days of hire.

Responsibilities

• Conduct individual and group therapy sessions for patients in the PHP.
• Participate in treatment planning, discharge, transition, and after-care processes.
• Provide family therapy sessions as needed to support continuity of care.
• Support with patient vitals and urine drug screening as needed.
• Communicate with patients, families, and referral sources in compliance with HIPAA and 42 CFR Part 2.
• Collaborate with interdisciplinary teams including physicians, UR, and nursing staff.
• Support UR team functions and provide treatment updates or documentation for authorizations.
• Complete clinical paperwork including psychosocial assessments, suicide risk scales, and progress notes.
• Respond to patient crises and participate in de-escalation procedures.
• Maintain confidentiality and comply with HIPAA and clinical regulations.
• Assist care coordination with discharge planning and continuity of care.
• Complete all documentation thoroughly and on time.
