Job summary

Woodland Springs is seeking a Therapist for the Partial Hospitalization Program (PHP) to deliver individual and group therapy, participate in discharge planning, and coordinate care with interdisciplinary teams in Conroe, TX. You will treat patients with psychiatric, emotional, and/or substance use disorders while ensuring HIPAA and regulatory compliance.

The role requires a Master's degree in Social Work, Counseling, or equivalent and a current license, with CPR and de-escalation certification

Qualifications

• Master's degree in Social Work, Counseling, or equivalent required.
• Current clinical or social work license as required by state regulations.
• CPR and De-escalation certification required or obtain within 30 days of hire.

Responsibilities

• Conduct individual and group therapy sessions to educate and treat patients in the PHP.
• Actively participate in treatment planning, discharge, transition, and after-care processes.
• Provide family therapy sessions to support continuity of care.
• Support with patient vitals and urine drug screening as needed.
• Communicate with patients, families, and referral sources per HIPAA and 42 CFR Part 2.
• Collaborate with interdisciplinary teams including physicians, UR, and nursing staff.
• Assist UR team functions and provide treatment updates or documentation for authorizations.
• Complete psychosocial assessments, Columbia Suicide Risk Scale, and progress notes.
• Respond to patient crises and participate in de-escalation procedures.
• Maintain confidentiality and comply with clinical regulations (TJC, CARF, Medicaid).
• Assist care coordination with discharge planning and continuity of care.
• Complete documentation thoroughly and within facility timelines.
• Perform additional duties as assigned by leadership.
