Job summary

HealthDrive is seeking a Licensed Independent Clinical Social Worker (LICSW) for a part-time position (2 days per week) serving residents in nursing facilities across the South Shore/Norfolk County, MA area, with work available in Plymouth and Franklin. The role offers professional autonomy and a strong patient base with comprehensive administrative support.

This on-site position includes flexible scheduling, no evenings or weekends, and competitive pay prorated to schedule.

Qualifications

• Master's degree from an accredited college or university.
• State LICSW license is required.
• Valid driver’s license.
• Ability to work independently on a daily basis.
• Excellent written, verbal, interpersonal and organizational skills.
• Ability to use email, the internet, and healthcare software.

Responsibilities

• Complete psychosocial (initial) assessments.
• Provide individual and group therapy.
• Develop behavior plans for residents.
• Recommend behavioral/therapeutic interventions to nursing home staff.
• Enter patient information and required clinical documentation in EMR system.
• Perform other duties as assigned; travel daily to skilled nursing facilities.

Responsibilities

The Therapist will provide behavioral health services to the residents of nursing homes, assisted living facilities, and long-term care facilities within an assigned territory.

Essential Functions:

• Complete psychosocial (initial) assessments
• Provide individual and group therapy
• Develop behavior plans for residents
• Recommend behavioral/therapeutic interventions to nursing home staff
• Enters patient information and required clinical documentation in EMR system
• Perform other duties as assigned
• Mobile model treatment of patients requires daily travel to skilled nursing facilities
