Job summary

Health Connect America is seeking a licensed therapist to provide high-quality services to children, families, and adults across our multi-state footprint. You will conduct biopsychosocial assessments, plan treatment, and deliver individual, family, and group therapy with a multidisciplinary team.

Qualifications

• Master's Degree in Social Work, Counseling, or a related field required.
• Must be eligible for licensure as an LPC, LMFT, or LCSW.
• Experience conducting behavioral health assessments and treatment planning.
• Knowledge of evidence-based treatment approaches and crisis intervention.

Responsibilities

• Conduct comprehensive biopsychosocial assessments and utilize evidence-based assessment tools to identify client strengths, needs, and treatment recommendations
• Develop individualized treatment plans with measurable goals and objectives in collaboration with clients, families, and multidisciplinary treatment teams
• Provide individual, family, and group therapy using evidence-based interventions to address anxiety, depression, trauma, behavioral challenges, and other mental health conditions
• Monitor client progress, maintain ongoing communication and follow-up, and adjust treatment interventions to support positive outcomes
• Provide crisis intervention, safety planning, and referrals to additional resources and support services as needed
• Maintain timely, accurate clinical documentation while ensuring compliance with organizational policies, payer requirements, accreditation standards, and state regulations
