Qualifications

Responsibilities

• Provide individual counseling and/or group facilitation to clients with mental health and substance use disorders.
• Maintain concise, timely documentation in the electronic health record.
• Conduct discharge planning based on patient needs.
• Demonstrate a holistic approach to care including family/support system when appropriate.
• Attend required internal/external trainings on evidence-based practices for Behavioral Health.
• Provide crisis intervention and support under supervision.
• Participate in treatment team and staff meetings.
• Develop networks of care for client welfare.
