Job summary

Health Care for the Homeless in Baltimore is seeking a Therapist Case Manager to provide behavioral health assessments, individual and group therapy, and intensive case management to clients facing housing instability.

You will conduct home visits, coordinate care with interdisciplinary teams, and document progress in the electronic health record. A Master’s degree in Social Work (LMSW or LCSW-C) and relevant therapy/case-management experience are required.

Qualifications

  • Master’s degree in Social Work (LMSW or LCSW-C required).
  • 1 year of therapy and case management experience (substance use/mental health preferred).
  • Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.

Responsibilities

  • Deliver behavioral health assessments, individual and group therapy, and intensive case management.
  • Conduct home visits and community visits to monitor client progress and well-being.
  • Collaborate with interdisciplinary teams and external partners to coordinate care.
  • Maintain thorough and timely documentation in the electronic health record.

Key Responsibilities
  • Deliver behavioral health assessments, individual and group therapy, and intensive case management.
  • Conduct home and community visits to ensure client progress and well-being.
  • Collaborate with interdisciplinary teams and external partners to coordinate care.
  • Support clients with budgeting and financial literacy as part of representative payee services.
  • Maintain thorough and timely documentation in the electronic health record.
What We’re Looking For
  • Master’s degree in Social Work (LMSW or LCSW-C required).
  • At least 1 year of experience in therapy and case management for individuals with substance use and mental health disorders (preferred).
  • Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.
  • Strong communication, organizational, and crisis intervention skills.
  • Valid Maryland driver’s license and access to a personal vehicle.
