Job summary

Personalized Health Partners is seeking a Therapist/Behavioral Health Care Manager to provide psychotherapy and group therapy to residents in long-term care settings. Licensed professionals will coordinate with psychiatrists and primary care providers to deliver comprehensive behavioral health care.

The role requires a Master's degree and a licensed credential (LPC/LPCC/LISW/LISW-S). Full-time and part-time options are available with flexible scheduling and ongoing professional development

Qualifications

Masters Degree required.
Licensed as LPC, LPCC, LISW, or LISW-S.
Experience with diverse patients incl. chronic severe mental illness.

Responsibilities

Deliver individual and group psychotherapy sessions tailored to residents.
Facilitate collaborative care management with psychiatrists, PCPs, and team members.
Address mental health needs of residents with chronic severe mental illness and neurocognitive disorders.
Develop and implement therapeutic interventions using evidence-based modalities.
Maintain detailed EMR documentation.
Conduct initial behavioral health evaluations as needed.
