Job summary

CommuniCare Health seeks Social Workers and Counselors to join as Therapist/Behavioral Health Care Managers in Dayton, OH. Full-time and part-time options are available, with licensure as LSW/LISW/LPC/LPCC required. Provide psychotherapy and coordinate care within a multidisciplinary team.

Responsibilities include individual and group therapy, collaboration with psychiatrists and PCPs, and maintaining EMR documentation for residents with chronic mental illness and neurocognitive disorders.

Qualifications

• Master’s degree in Social Work or Counseling is required.
• Licensed as LSW/LISW/LPC or LPCC for the role.
• Experience with diverse populations and neurocognitive disorders preferred.
• Proficient with electronic medical records (EMR) systems.

Responsibilities

• Provide individual and group psychotherapy sessions for residents.
• Coordinate collaborative care with psychiatrists and primary care providers.
• Address mental health needs of residents with chronic illness and disorders.
• Maintain detailed clinical documentation in EMR.
