Job summary

Personalized Health Partners is seeking Therapist/Behavioral Health Care Managers to provide psychotherapy and collaborative care management for residents in long-term care settings. A Master’s degree and licensure as LSW/LISW/LPC/LPCC/LISW-S are required with EMR proficiency.

The role includes individual and group therapy, coordination with psychiatrists and primary care providers, and documentation in electronic records.

Qualifications

• Master's Degree in Social Work or Counseling from an accredited institution is required.
• Licensed as LSW/LISW/LPC/LPCC/LISW-S is required.
• Experience with diverse patient populations and chronic mental health conditions preferred.
• Familiarity with EMR systems and evidence-based modalities is advantageous.

Responsibilities

• Provide individual and group psychotherapy to residents.
• Coordinate collaborative care with psychiatrists and primary care providers.
• Conduct behavioral health evaluations and document in EMR.
• Develop and implement evidence-based therapeutic interventions.
• Collaborate with a multidisciplinary team to meet resident needs.
