Job summary

DCCCA is seeking a full-time case manager to provide safety, stability, and well-being support for children and families during referrals. You will deliver strengths-based, culturally competent case management, assess high-acuity needs, and coordinate placement with foster homes to best match the child's needs and background.

Qualifications

• Requires a bachelor's degree in a human services field.
• Requires at least two years of child welfare and/or family services experience.
• Must possess a valid Driver's License, have in force automobile liability insurance coverage, and maintain a Motor Vehicle Record (MVR) that meets the organization's insurability standard as defined by DCCCA policy.

Responsibilities

• Provides for the safety, stability and well-being of children and families during the referral period.
• Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models.
• Assesses the needs of children/youth with high acuity needs to ensure appropriate placement and support services.
• Facilitates the placement of children into suitable foster homes, ensuring a match that considers the child's needs, cultural background, and the foster family's capabilities.
• Provides ongoing support and guidance to families, offering training and resources to promote a stable nurturing environment.
