Job summary

Concierge Home Care in Ft. Walton Beach, FL is seeking a Home Health Account Executive to grow census in the Okaloosa County territory. This full-time, territory-based role offers autonomy, a performance-based bonus, and growth opportunities.

You will leverage an existing book of business to build referral relationships, coordinate services, and collaborate with clinicians while traveling within the territory.

Qualifications

• Active book of business within the territory (REQUIRED).
• Healthcare sales experience preferred.
• Clinical background (RN, LPN, MSW, etc.) preferred.
• Excellent communication, presentation, and relationship-building skills.
• Must be able to travel within the territory.
• Valid driver’s license and reliable transportation required.
• Ongoing mentorship and leadership support.
• CEUs provided during tenure.

Responsibilities

• Build and maintain strong referral relationships.
• Drive census growth by meeting/exceeding sales goals.
• Identify resident care needs and coordinate appropriate services.
• Act as a resident advocate to ensure smooth transitions to home care.
• Manage the referral-to-admission process efficiently.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Provide education to residents, caregivers, and community partners.
• Maintain consistent communication with executive leadership and referral sources.
