Job summary

Tom Trese Networking LLC is seeking a Clinical Manager to oversee patient care services and supervise clinical staff in a Home Health setting in California. The role emphasizes CHAP compliance, QAPI leadership, and survey readiness, with a focus on safe, effective care.

The manager will coordinate patient care, lead quality improvement initiatives, and ensure readiness for regulatory surveys across multiple locations. Travel to business sites is expected as part of the role.

Qualifications

• Graduate from approved school of professional nursing and currently licensed to practice as an RN in the state of operation.
• 5+ yrs RN experience with 2+ yrs supervisor experience in Home Health.
• Experience managing RN services in the home, community, or clinic is preferred.
• Licensed to practice as an RN in the state of operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with insured vehicle and be able to travel to all locations.

Responsibilities

• Oversee day-to-day clinical and administrative office operations.
• Direct care delivery to align with professional standards, policies, and regulations.
• Serve as acting leader in the absence of the Territory Administrator and cover PCM/CSS roles.
• Manage the intake and referral process and collaborate with an interdisciplinary team to develop care plans.
• Evaluate care quality and coordinate transfers and discharges with clinicians and physicians.
• Review discharge data and oversee Discharge OASIS forms.
• Direct QAPI initiatives and track incidents with corrective actions.
• Maintain accreditation readiness and manage HHCAHPS survey distribution across SHP.
• Lead Emergency Preparedness Planning and ongoing event monitoring.
