Job summary

African American Planning Commission in Brooklyn, NY seeks a Case Manager to conduct intake, assess eligibility, and develop tailored plans for residents in Glenmore Manor. You will coordinate care, advocate for services, and help tenants pursue employment and education while maintaining accurate records.

With a Bachelor’s degree and 3–5 years in residential settings, you’ll work under the Assistant Program Director, ensuring confidentiality and outcomes reporting to support tenant independence

Qualifications

• Bachelor’s degree in a related field (social work, psychology, sociology, etc.) with 3–5 years of residential experience.
• Experience delivering tenant-centered, outcome-oriented services.
• Strong written and verbal communication and confidentiality practices.

Responsibilities

• Conduct intake and eligibility assessments for program entry.
• Develop and update support plans and case notes.
• Coordinate care with social services and collateral contacts.
• Advocate for tenants to access medical, mental health, and housing services.
• Maintain tenant files with accuracy and confidentiality; track outcomes.
• Engage tenants in life skills, employment, education and other activities.
