Center City Housing Corp is seeking a Case Manager in Minnesota to provide daily tenant advocacy and support, helping tenants access health, social services, counseling, employment, and financial benefits. You will document progress and work with the Site Director to collect tenant data and ensure timely completion of tasks.

The role offers a full-time schedule with benefits including health, dental, life, long-term disability insurance, paid holidays, vacation, 401k, and sick time.

Competenze

• Degree in related field or equivalent experience working with population.
• Valid driver’s license and dependable transportation.
• Minnesota Background Study clearance required.
• Ability to read, analyze, and complete documentation for government agencies.
• Confidentiality and ability to work with diverse tenants and staff.

Mansioni

• Provide daily tenant advocacy and support.
• Identify and encourage goal completion.
• Maintain progress notes and records.
• Assist tenants to obtain health, social services, counseling, employment, and financial benefits.
• Teach or refer for needed independent living skills.
• Coordinate with Site Director on tenant data collection.
• Prioritize and complete duties efficiently.
• Communicate with individuals and groups, advocate for tenants, document progress notes.
• Maintain professional relationships with colleagues and community agencies.
