Royal Oaks in Duarte, CA, part of HumanGood communities recognized as a Best CCRC-2026, is seeking a Temporary Social Services Designee to join its Health Center team. Under minimal supervision, the Social Services Designee assesses resident social service needs and coordinates care with the interdisciplinary team.

The role emphasizes advocacy for residents and families, strong communication, and connecting them to community resources; compensation is $30.00-$35.00 per hour DOE, with benefits

Qualifications

• Bachelor's degree in Social Work preferred.
• Minimum of two years of experience working with older adults in a long-term care setting.
• Social Services certification desired.
• Strong communication, problem-solving, and interpersonal skills, with knowledge of community resources.

Responsibilities

• Assessing residents' social service needs and providing compassionate support to enhance their well-being and quality of life.
• Coordinating resident care plans, admissions, discharges, and transitions in collaboration with the interdisciplinary team.
• Supporting residents and families by addressing concerns, resolving grievances, and connecting them to appropriate community resources.
• Maintaining accurate social service assessments, progress notes, and documentation while advocating for residents' rights and their individual needs.
