Job summary

Royal Oaks in Duarte, CA, part of HumanGood, seeks a Temporary Social Services Designee to support residents on the Health Center team.

Under minimal supervision, you assess social service needs, coordinate care plans, and advocate for residents and families, ensuring timely access to community resources.

This on-site role offers hourly pay $30.00–$35.00 with DOE and benefits for part-time and full-time team members.

Qualifications

• Bachelor's degree in Social Work preferred.
• Minimum of two years of experience working with older adults in long-term care.
• Strong communication, problem-solving, and interpersonal skills with knowledge of community resources.

Responsibilities

• Assess residents' social service needs and provide compassionate support to enhance well-being and quality of life.
• Coordinate resident care plans, admissions, discharges, and transitions with the interdisciplinary team.
• Support residents and families by addressing concerns, resolving grievances, and connecting them to community resources.
• Maintain accurate social service assessments, progress notes, and documentation while advocating for residents' rights and needs.
