Qualifications

Royal Oaks in Duarte, CA, one of several HumanGood communities recognized as a Best CCRC-2026 by U.S. News & World Report, is seeking a Temporary Social Services Designee to join its Health Center team. Under minimal supervision, the Social Services Designee assesses resident social service needs and ensures the identified needs are addressed. Advocates for the resident and family while maintaining a professional and positive customer service approach.

Temporary **

Shift: 8:00 a.m. - 4:30 p.m. Monday - Friday

Compensation: $30.00 - $35.00 per hour (DOE)

A day in the life may include:

• Assessing residents' social service needs and providing compassionate support to enhance their well-being and quality of life.
• Coordinating resident care plans, admissions, discharges, and transitions in collaboration with the interdisciplinary team.
• Supporting residents and families by addressing concerns, resolving grievances, and connecting them to appropriate community resources.
• Maintaining accurate social service assessments, progress notes, and documentation while advocating for residents' rights and their individual needs..

To be successful in the role, you would have:

• Bachelor's degree in Social Work preferred
• Minimum of two years of experience working with older adults in a long-term care setting
• Strong communication, problem-solving, and interpersonal skills, with knowledge of community resources
