Under minimal supervision, you assess resident social service needs and ensure they are addressed, advocating for residents and families with professional customer service.

The role emphasizes collaboration with the interdisciplinary team to coordinate care plans, admissions, discharges, and transitions, while documenting assessments and progress notes.

Qualificações

• Bachelor's degree in Social Work preferred.
• Minimum of two years of experience working with older adults in a long-term care setting.
• Strong communication, problem-solving, and interpersonal skills, with knowledge of community resources.

Responsabilidades

• Assess residents' social service needs and provide compassionate support to enhance well-being and quality of life.
• Coordinate resident care plans, admissions, discharges, and transitions with the interdisciplinary team.
• Support residents and families by addressing concerns, resolving grievances, and connecting them to community resources.
• Maintain accurate social service assessments, progress notes, and documentation while advocating for residents' rights and individual needs.
