Job summary

RCM Health Care Services seeks a Full-Time Temporary Field Registered Nurse in Bronx, NY to support the CAPABLE program. The role involves home environment assessments, coordinating with OTR, and developing plans to improve safety and independence for participants.

The RN will conduct comprehensive evaluations, document findings in the EMR, educate participants and caregivers, and maintain compliance with NYSDOH CAPABLE regulations while traveling to participant homes within the service area.

Qualifications

Graduate of an accredited nursing program.
Current, active NY RN license.
Minimum 2 years in home health care/CHHA and community health.

Responsibilities

Conducts comprehensive assessments in the home environment identifying factors that limit overall functional status, independence and quality of life.
Evaluates participant’s physical, cognitive, emotional and sensory abilities.
Assesses ADLs and provides recommended interventions.
Maintains accurate, timely, and compliant medical records per NYSDOH CAPABLE regulations.
Coordinates with CAPABLE team and OTR to support quality improvement initiatives.
Educates participants and caregivers on disease management and safety protocols.
