Qualifications

• Licensed and registered NY RN required.
• Bachelor’s Degree in Nursing preferred.
• 1–2 years nursing experience in acute care.
• 1 year experience in home care.
• EMR knowledge.
• Driver's license and a vehicle required.

Responsibilities

• Assesses newly referred clients and eligibility for home care.
• Provides patient and family teaching prior to discharge and ongoing.
• Performs in-home patient assessments and documents in EMR timely.
• Provides skilled nursing per medical orders and agency policies.
• Communicates with referrers and discharge planners to review eligibility and plan of care.
• Assists referring agents with alternate plans for ineligible clients.
• Develops and implements treatment plans with patient’s medical practitioner.
• Creates and discusses the home health aide plan of care with the assigned aides.
• Supervises involved disciplines and evaluates aides' performance.
• Performs other nursing activities as directed.
