Job summary

Providence Strategic And Management Services in Seattle seeks a Clinical Program Manager to own Post Acute programming across the North Division. You will develop and operate programs that move patients to post-acute settings, working closely with external partners and internal case management teams.

You will leverage your case management and project management experience to implement programs with leadership, while aligning with Providence's mission and patient-centered care across our system.

Qualifications

• Bachelor's degree in a related field required.
• 8 years of experience in Case Management (Care Coordination or Utilization Management).
• 1 year of Project Management experience.

Responsibilities

• Operational owner for Post Acute programming across the North Division.
• Develop and operate programs that support patient throughput to post-acute settings.
• Partner closely with external partners and entities.
• Utilize case management experience to facilitate program implementation within case management teams and leadership.
