Job summary

Texas Health and Human Services Commission (HHSC) seeks a Community Care Worker III in Corpus Christi to support Medicaid 1115 waiver programs and LTSS across CCSE and MCO interfaces. This role focuses on eligibility, transition planning, and service coordination with care providers and ME PD staff.

Responsibilities include processing waivers, validating eligibility, coordinating transitions from nursing facilities to community settings, and assisting clients with waivers while maintaining

Qualifications

• High School Diploma or GED required.
• Preferred experience with STAR+PLUS HCBS, MDCP, LTSS and/or MEPD services.
• Proficiency with Outlook, Word and Excel; basic TIERS/SAS knowledge.
• Ability to communicate effectively with diverse stakeholders.

Responsibilities

• Assist with Medicaid 1115 waiver program processes and eligibility.
• Coordinate transitions from nursing facilities to the community (Promoting Independence).
• Enter and validate waiver eligibility in enrollment systems and notify key parties.
• Support enrollment and MCO authorization, track appeals and hearings.
