Qualifications

Overview

We’re looking for a Telephonic Triage Nurse to join our team. You will directly report to the Telephonic Triage Manager. This is a remote position and you will be responsible to assess callers' symptoms, prioritize calls, and advise patients or families in basic treatment or make the critical decision to send a nurse to evaluate the patient’s current situation.

The shift is Sat-Sun 8am-5pm and Mon-Tues 5pm-11pm.

Responsibilities
  • Receives and responds to calls for assigned Hospice service area
  • Collects appropriate data in order to make informed decisions related to patients' care
  • Prioritizes various patient care situations, providing information, education, support, and continuity of care in relation to needs identified on the Plan of Care to the patient’s family and/or significant others via the phone or assigns on-call RN or other appropriate discipline to complete an in-person patient visit
About You
  • Graduate of an accredited school of nursing
  • Current Registered Nurse (RN) licensure within a NLC Compact State
  • Three (3) years clinical experience
  • Must possess a minimum of two (2) years’ experience, either in oncology, long-term care, or hospice or home health
  • Prior experience with telephone triage
  • Prior experience using Electronic Medical Records (EMR) and Microsoft Office Suite including Word and Outlook
