Qualifications

• Bachelor’s degree social work or related field required
• MSW or LMSW preferred

• Working knowledge of Medicare and Medicaid regulations preferred
• Current LMSW in New York State preferred

Responsibilities

• Provides telephonic support for members’ medical, psychosocial, physical and spiritual needs
• Develops, implements, and monitors person-centered service plans
• Ensures continuity of care across healthcare settings by collaborating with the interdisciplinary team, PCP, and member representatives
• Educates members on disease processes, preventative health, and quality of life interventions
• Adheres to CMS/DOH regulations and policies
