Qualifications

• Minimum 2 years healthcare experience with LTSS or related field.
• Must reside in Texas; telephonic role; valid driver's license and transportation.
• Proficient in Microsoft Office including Outlook, Excel, Teams, and OneNote.

Responsibilities

• Conduct comprehensive member assessments within required timelines.
• Coordinate waiver enrollment and disenrollment processes.
• Develop and implement care plans with the care team.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of services including behavioral health and LTSS.
• Coordinate with ICT meetings for service approvals and collaboration.
• Travel 25-40% as required by contracts.
