Qualifications

• 2+ years health care experience including LTSS/care management.
• License/certification required only if state contract mandates it.
• Bachelor's degree may be required in some states.
• Valid driver's license and reliable transportation for travel.
• Familiar with community resources and diverse populations.
• Proficient in Microsoft Office and online portals.

Responsibilities

• Completes comprehensive member assessments within timelines, including home visits as required.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians, and staff.
• Monitors care plan effectiveness and updates as needed.
• Promotes integration of behavioral health and LTSS services.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for approvals/denials and collaboration.
• Uses motivational interviewing and Molina guideposts in member contact.
• Addresses psychosocial, financial, and medical barriers to care.
• Identifies critical incidents and develops preventive plans.
• Collaborates with licensed care managers as needed.
• 25-40% local travel may be required.
