Job summary

Commonwealth Care Alliance in Massachusetts seeks a Care Delivery Triage RN to perform telephonic clinical triage and provide health information and care coordination as a key member of the primary care team.

You will serve as the primary telephonic contact for care coordination, educate patients on chronic disease management, collaborate with APC/physician and Care Partnership, and help arrange referrals, tests, and follow-up while maintaining accurate documentation.

Qualifications

• Associates degree in nursing required.
• Bachelor's degree in nursing preferred.
• Three (3) years of RN experience in a high touch clinical environment or home care.
• Two (2) years of experience caring for patients with complex medical, behavioral health, and social needs.

Responsibilities

• Serve as primary telephonic contact for clinical and care coordination needs, documenting promptly and accurately.
• Evaluate and assess health care needs using triage protocols and workflows.
• Escalate issues to APC/physician based on disposition and nursing judgment.
• Coordinate and schedule appointments based on triage disposition or care team requests.
• Provide telephonic education on chronic disease management and preventive health maintenance.
• Coordinate referrals, specialty visits, testing, social services, and skilled services.
• Communicate lab results and medical advice with direction from APC/physician.
