Job summary

CareSource in Massachusetts is seeking a Telephonic Care Delivery Triage RN to support the primary care team by performing clinical triage, providing health information, and coordinating care with internal and external teams.

You will serve as a liaison for the Care Partnership, manage referrals, educate patients on chronic disease management, and participate in vaccination and care initiatives using telehealth tools to deliver timely, patient-centered care.

Qualifications

• Associates degree in nursing required.
• Bachelor's degree in nursing preferred.
• Three years of RN experience in a high-touch clinical environment or home health setting required.
• Two years of experience caring for patients with complex medical, behavioral health, and social needs required.

Responsibilities

• Serve as primary telephonic contact for clinical and care coordination needs.
• Evaluate and triage health care needs using established protocols.
• Escalate issues to APC/physician based on triage protocol or nursing judgment.
• Coordinate and schedule appointments based on triage disposition or care team requests.
• Provide telephonic education to patient and family regarding chronic disease management.
• Coordinate referrals, specialty visits, testing, social services, and skilled services.
• Communicate lab results and medical advice with direction from APC or physician.
• Assist with vaccination planning and other clinical initiatives.
• Attend team meetings and support other nursing areas as needed.
• Perform other job-related duties as requested.
