Job summary

Patient First is seeking a Telephonic Care Manager to serve as a resource to clinical teams and patients across the practice. The role focuses on developing and maintaining relationships with physicians, defining quality measures, and coordinating care across multiple providers to close gaps in care.

Ideal candidates hold RN licensure (BSN preferred), with 3+ years in medical/surgical, ER, or ambulatory care.

Qualifications

• RN licensure required; RN, BSN preferred.
• Must be licensed in all states Patient First operates.
• Current CPR, ACLS, and PALS certifications.
• Minimum of three years’ clinical experience in medical/surgical, ER, critical care, and ambulatory care preferred.
• Telephonic triage, coaching, or care coordination experience required.
• Ability to work in a team-oriented environment; highly organized and well-developed oral, written, presentation, and interpersonal communication skills.

Responsibilities

• Serving as a telephonic care management resource to clinical teams and developing strong relationships with Patient First Physicians.
• Defining quality measures, establishing goals, and developing protocols using evidence-based care measures.
• Reviewing patient records to identify gaps in care and coordinating strategies to close gaps.
• Educating patients and care teams on preventative care and care coordination.
• Assisting in scheduling, referrals, and coordination of care across multiple providers.
