Responsibilities

• Review medical records and triage to the appropriate physician; collaborate with case managers.
• Coordinate scheduling of telehealth appointments with hospital clinics and physicians.
• Attend telehealth appointments and assist with follow-ups as requested.
• Identify areas for improvement and create new processes and procedures as needed.
• Provide administrative, regulatory, and operational support for the telehealth program.
• Report to the Manager, Access Services; manage telehealth inquiries and requests.
• Serve as the primary contact for customers and communicate status and progress.

Our client, a Medical Center facility under the aegis of a California Public Ivy university and one of largest health delivery systems in California, seeks an accomplished Telehealth Patient Access Coordinator.

Notes:

• Onsite role.
• Work Hours: Mon – Fri, 8:00am – 5:00pm.

Description:

• In this role, you will work independently and facilitate access to care for patients who seek telehealth appointments by processing inquiries and converting them to patients for hospital.
• This involves but is not limited to obtaining required demographic information, insurance information, and medical reports, as needed.
• You will register patients in Care Connect and Salesforce, the department's online CRM, organize and upload medical records to Epic via ICAP.
• Review medical records, and triage cases to the most appropriate physician at hospital; collaborate with case managers, as needed.
• Outreach hospital clinics and physicians and coordinate scheduling of telehealth appointments.
• Attend telehealth appointments and assist with any requested follow ups.
• Identify areas of improvement, and create new processes and procedures as needed.
• Provide administrative, regulatory, and operational support for the organization’s telehealth program offerings.
• Reporting directly to the Manager, Access Services, Telehealth Coordinator is responsible for managing telehealth inquiries and requests.
• Serve as the customer’s primary point of contact, document and communicate status and progress.
• This role will collaborate with other members of Patient Navigation and Business Services as well as members within hospital, and partner with local clinical and operational leaders and international partners.

Qualifications:

• Previous clinic experience preferred minimum 6 months.
• Care Connect experience.
