Job summary

MindCare Solutions Group is seeking a Telehealth Care Navigation Coordinator for PRN and holiday coverage. The role acts as a liaison between MindCare treatment teams and partner sites, coordinating schedules, clinical activities, information exchange, and consultations through telemedicine.

Responsibilities include facilitating clinical consultations, ensuring timely reporting of records and billing, and operating telemedicine and EMR systems.

Qualifications

Responsibilities

• Facilitate safe and effective clinical consultations.
• Coordinate with physicians and client staff to ensure timely reporting of medical records, schedules, billing, utilization, and outcomes.
• Manage and coordinate schedules and appointments, including pre-appointment chart prep and delivery of final notes to customers.
• Operate video conferencing, telemedicine platforms, and EMR systems.
• Assist sites with scheduling to optimize clinical time and communicate provider availability changes.
