Job summary

ChenMed is seeking an Advanced Practice Provider (APP) I, CareLine, to deliver diagnostic care through virtual consultations. You will assess patients, document findings, and develop care plans while serving as the on-call clinical lead after hours and weekends.

You will collaborate with PCPs, other providers, and care teams to achieve optimal outcomes. Travel to applicable states may be required, with emphasis on compliant telemedicine practices.

Qualifications

• BSN and APN certification required with graduate nursing degree.
• Master’s degree in Nursing required.
• Board certification by AANP or ANCC.
• BLS certification required on hire.
• Multi-state licensure to include GA, VA and at least two more states.

Responsibilities

• Assess acute and non-acute clinical problems via virtual visits.
• Develop, document and implement patient management plans.
• Collaborate with PCPs and other providers to report health data.
• Coordinate telehealth visits and ensure regulatory compliance.
• Maintain high-quality telehealth practices using data for improvement.
