Job summary

ChenMed is seeking an Advanced Practice Provider I, CareLine to deliver diagnostic care primarily through virtual consultations. You will function as dispositional authority for after-hours/weekend calls and document patient health statuses using telehealth tools.

The role requires state licensure in FL with multi-state capability, MSN/ANP certification, and extensive telemedicine experience. Weekend/evening hours and travel as needed are expected.

Qualifications

• Bachelor’s degree in Nursing (BSN) and graduate of a nursing program with specialty certification.
• Master’s degree in Nursing required.
• Board certification by AANP or ANCC required.
• Multi-state licensure to include FL and at least 2 other states.
• DEA number may be required depending on state practice.

Responsibilities

• Assess acute and non-acute problems via telemedicine and document care plans.
• Plan care considering patient background and advocate for patients.
• Collect health data and analyze for maintenance/improvement.
• Report health data to PCP and other providers to ensure guideline adherence.
• Collaborate with on-call PCP to achieve outcomes via telemedicine.
• Contribute to telehealth/telemedicine policy and quality programs.
• Communicate using various formats to build relationships and deliver care.
• Adhere to policies and a formal practice agreement.
• Participate in quality improvement activities and meetings.
• Perform other duties as assigned.
