ChenMed in south Florida seeks an Advanced Practice Provider (APP) for Telehealth, PRN coverage. The role delivers diagnostic patient care through virtual visits and coordinates after-hours and weekend calls.

You will assess, diagnose, and document geriatric patients' health status via telemedicine, with a commitment to perform 4 weekend hours per week and at least 24 weekend hours per month to maintain per diem status.

Qualifications

• BSN required; advanced practice nursing certification.
• MSN degree required; APRN with specialty certification.
• Active RN license; board certification (AANP/ANCC) required.
• Multistate licensure including Florida.
• Minimum 3 years acute/primary/urgent care; geriatric experience preferred.
• Telehealth experience preferred; weekend commitment as needed.

Responsabilités

• Assess acute and non-acute problems via video or phone and document histories.
• Develop and implement a patient management plan.
• Provide education and referrals as needed.
• Cover nights, weekends, and holidays; at least 24 weekend hours monthly.
• Document data clearly; communicate with PCP and providers.
• Collaborate with on-call PCP to achieve outcomes; telemedicine focus.
