Job summary

ChenMed is seeking an Advanced Practice Provider for CareLine, PRN telehealth coverage. The role involves evaluating geriatric patients via video or phone, documenting health data, and supporting after-hours care with weekend hours.

Minimum 160 hours/year, nights/weekends/holidays required, and 72 hours per 3 months on weekends. Must hold BSN, MSN in Nursing, RN license, and NP certification; multi-state licensure encouraged.

Qualifications

• MD/NP/PA telemedicine practice knowledge and judgment.
• Ability to work independently within telehealth guidelines.
• Knowledge of state/federal telemedicine regulations.

Responsibilities

• Assess acute and non-acute clinical problems via telehealth.
• Document patient histories and management plans.
• Collaborate with on-call PCP and other providers.
• Provide patient education and referrals as needed.
• Utilize telemedicine to achieve optimal patient outcomes.
